PHOTO, VIDEO, AND MEDIA RELEASE 1. Grant of permission. I grant My Special World®, a 501(c)(3) nonprofit corporation ("MSW"), and its directors, officers, employees, volunteers, contractors, agents, successors, assigns, and anyone acting with its permission, the irrevocable right and permission to photograph, film, video record, audio record, live stream, and otherwise capture the image, likeness, voice, name, and participation of the attendee named in this registration ("Participant") at or in connection with MSW events and activities. 2. Ownership and use. All resulting photographs, video, audio, and other recordings ("Media") are and remain the sole property of MSW. MSW may use, reuse, edit, crop, alter, combine with other material, reproduce, publish, republish, distribute, display, broadcast, and license the Media, in whole or in part, worldwide, in any and all media now known or later developed, including websites, social media platforms such as TikTok, Facebook, Instagram, YouTube, and LinkedIn, print materials, presentations, news media, grant applications, fundraising appeals, and advertising, for any lawful purpose including promoting MSW and its mission, in perpetuity. 3. No compensation. I understand and agree that no compensation of any kind is or will be due to me or to Participant for this permission or for any use of the Media, now or in the future. 4. No right of approval. I waive any right to inspect or approve the finished Media or any copy, caption, or other material that may be used with it, and any right to royalties or other consideration arising from its use. 5. Release of claims. I release and hold harmless MSW and all persons acting under its permission from any and all claims, demands, causes of action, and liability arising out of or related to the capture or use of the Media, including without limitation claims for defamation, libel, invasion of privacy, violation of any right of publicity or personality, false light, misappropriation, copyright infringement, or emotional distress. 6. Names. MSW will not publish Participant's full name together with their image without separate permission, although a first name or first name and last initial may be used. 7. Third parties and re-sharing. I understand that once Media is published online it may be viewed, copied, shared, or re-posted by third parties outside of MSW's control, and that MSW is not responsible for such third-party use. 8. Authority. I represent that I am the Participant, or the parent or legal guardian of the Participant with full authority to grant this release on their behalf, and that I have read and understood this release. 9. Revocation. This permission is ongoing. If I later ask MSW in writing to stop using new Media of the Participant, MSW will honor that request going forward, but I understand MSW is not obligated to recall, retrieve, or remove Media already published, printed, distributed, or in the possession of third parties.

TRANSPORTATION, PARTICIPATION, ASSUMPTION OF RISK, AND RELEASE OF LIABILITY Please read carefully. This is a release of legal rights. 1. Voluntary participation. Participation in My Special World® ("MSW") events, activities, and transportation is entirely voluntary. I have chosen for the attendee named in this registration ("Participant") to take part, and Participant may decline or stop participating at any time. 2. Transportation. MSW may arrange transportation to and from events, including by limousine, party bus, van, bus, or other vehicle, some of which may be provided, owned, or operated by third-party companies, contractors, or volunteers who are not employees of MSW. I authorize MSW to transport Participant to and from the event and I consent to that transportation. I agree to provide accurate pickup and drop off information, to have Participant ready at the arranged time, and to ensure that a parent, guardian, or authorized adult is present at pickup and at drop off if Participant requires supervision. 3. Assumption of risk. I understand that participation and transportation involve inherent risks, including but not limited to: vehicle travel and traffic accidents; slips, trips, and falls; dancing, music, crowds, flashing or bright lights, and loud sound; food service and possible exposure to allergens; contact with other participants, volunteers, and members of the public; exposure to illness, including communicable disease; property damage, loss, or theft; and, in rare cases, serious injury, permanent disability, or death. I knowingly, freely, and voluntarily assume all such risks, both known and unknown, and accept full responsibility for Participant's participation. 4. Supervision. MSW provides general supervision only. MSW does not provide one-to-one aides, nursing, medical, behavioral, or personal care services unless expressly agreed in writing in advance. If Participant requires one-to-one supervision or personal assistance, I am responsible for providing an appropriate caregiver who will attend and remain with Participant. 5. Health and disclosure. I represent that Participant is physically and medically able to participate, and that I have disclosed to MSW any medical condition, allergy, medication, mobility need, sensory sensitivity, dietary restriction, behavioral consideration, or other information relevant to Participant's safety. I will update MSW if any of that information changes. 6. Medical authorization. In the event of injury or a medical emergency, I authorize MSW and its volunteers to secure emergency medical treatment, transportation, or first aid for Participant, and I agree to be financially responsible for all costs of that treatment and transportation. I understand MSW does not carry medical or health insurance for participants. 7. Conduct and removal. I agree that Participant and accompanying guests will follow event rules and the reasonable instructions of MSW staff, volunteers, and drivers. MSW may, in its sole discretion and without refund or liability, decline, suspend, or end participation, or remove any person whose conduct is unsafe, disruptive, or harmful to others. 8. Release and waiver. To the fullest extent permitted by law, I, on behalf of myself, Participant, and our respective heirs, family members, personal representatives, executors, administrators, and assigns, hereby RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE MSW and its directors, officers, board members, employees, volunteers, contractors, drivers, transportation providers, event hosts, venues, sponsors, donors, and agents (collectively, "Released Parties") from any and all claims, demands, damages, losses, costs, expenses, and causes of action of any kind, whether known or unknown, arising out of or related to Participant's participation, transportation, or presence at any MSW event, including claims arising from the ordinary negligence of any Released Party. This release does not apply to gross negligence, recklessness, or willful or intentional misconduct, and does not waive any right that may not be waived under applicable law. 9. Indemnification. I agree to indemnify, defend, and hold harmless the Released Parties from any claim, loss, liability, damage, or cost, including reasonable attorney fees, brought by or on behalf of Participant or any third party, arising out of Participant's participation or conduct, or out of any inaccurate or incomplete information I have provided. 10. Personal property. MSW is not responsible for loss of, damage to, or theft of personal property, including phones, wheelchairs, mobility devices, clothing, and other belongings. 11. Photography. I understand that MSW documents its events and that Participant may appear in photographs or video, as addressed in the separate Photo, Video, and Media Release. 12. Authority and understanding. I represent that I am the Participant, or the parent or legal guardian of the Participant with full legal authority to sign this agreement on their behalf. I have read this document in full, or have knowingly chosen not to read it, I understand it is a release of legal rights and an assumption of risk, and I sign it freely and voluntarily. 13. Governing law and severability. This agreement is governed by the laws of the State of Arizona, without regard to conflict of law rules, and any dispute will be brought in the state or federal courts located in Maricopa County, Arizona. If any provision is found unenforceable, the remaining provisions remain in full force and effect. 14. Electronic signature. I agree that signing electronically, by drawing or typing my name, has the same legal effect as a handwritten signature, and I consent to receiving and signing this agreement electronically.
